hello and good morning Thursday one day before the weekend and it is frigid I ran outside I was like Googling this morning cuz I was too lazy and warm in my bed to see what temperature ferns die at and then I ran downstairs to go get them off my front lawn I caved I caved last night and put the heat on it was that was just stupid a time of year where we start to wonder why didn't the Pilgrims land in the Bahamas seriously so many better options why uh you've got Dr Dan Arnold right next to me today everyone say hi Dr Dan good morning Mark thanks for joining us this morning he's here from Veritas Direct Care and I first saw you guys you did an openhouse sort of Fall Fest at Everly and my husband has been talking about you Non-Stop and then my friend Sue who works in your office she's our lab manager so Sue was like oh I was coming for uh my my yearly calendar Sue is an amazing photographer on Cave Cod she's got this awesome calendar uh good morning oh there's Sue right now so I was like oh I'm getting nervous we're counting down the months and Dr Dan Arnold thank you very much for being the calendar delivery guy welcome so talk to everybody about what exactly it is that you do because I think you're needed more now than ever before right well I would say I don't know about that Primary Care is needed now more than ever before um so I am a primary care physician uh I've been practicing on the cape uh in various different ways is for the last 14 years um and I'm here I I truly believe in Primary Care yeah uh Primary Care is important it is uh necessary it is vital for not just a single patient but for a community um I also believe that Primary Care should be sustainable as a practice model and as a business model and uh in 2018 I started my own practice Primary Care Cape Cod uh because I also enjoy practicing medicine the way I want to practice the way I believe it should be practiced I want to do what's right for the patient sidebar Dr Dan has six children so he has made all of these people still alive so I feel like that's a good track record right there it starts at home right they're they get dressed and they're fed that's a victory yes did it and you have five so we definitely know what it's like I mean you can handle high stress situations no but I feel like primary care has definitely gotten away from the I watched the stupid Hallmark movies by the way they started like three weeks ago if you're just not even noticing your Christmas fair but the town doctor and you went to the town doctor for all of your ailments and they knew you so they knew when you were off they knew when something was was aai they they knew your behavior they knew everything about you right now I feel like we peace me our healthare because it's unaffordable it's I'm just going to go to the specialist right away and those people are I feel like and I'm just speaking from me not Dr Dan's perspective it's like all right we're treating the problem get the hell out of my office never want to see you again next no I I agree and primary care when it is successful is when you have a uh a relationship the relationship between the patient and the healthcare provider is set and you're right when Primary Care is successful I say to patients all the time when we know our patients so well that within two steps of walking in the room I know whether or not they're sick or healthy uh you also want to set up a relationship with the patient wants to set up a strong relationship with their health care provider so that there's a trusting relationship so if you come to see me and one day you're ill you're going to want to trust my guidance yeah and you're right in in the the system that has grown in this country over the last few decades we have to go so fast fast fast we don't have the time to BU to build and focus on that relationship and yes it leads to more tests it leads to more referrals it leads to more Specialists and increased costs so it is my opinion that Primary Care needs to slow down we need to spend more time with our patients really truly get to know our patients and that's what we're doing in our new practice where did you decide to come up with this model I love it I'm just waiting my feet into entrepreneurial but honestly you you went to Meda school school like you're supposed to follow that line of I became a doctor and now I'm supposed to go work here and what was broken for you that made you decide this is just not working for anyone um so running my own business yeah it's you know nothing like having to put food on the table for six kids and also making payroll every other week well and I'm sure you've got college loans too that are a lot larger than other people's I I do I have a a decent sized med student loans worth every penny but uh but um I didn't in invent direct primary care direct Primary Care is the model that we are following in this practice direct Primary Care was born in the mid to late 1990s um some Intrepid Physicians who were responding to the birth of the HMO movement in this country the HMO movement was a well-intended um accident trying to help to lower costs in this country but all it did was add more administrative burden to there's a problem here we all understand that there's a problem here and so much of our pay is going towards it and it never seems like it's enough and the the the fundamental problem is you take a patient and a physician put a trillion dollar industry in between us that's where we're pouring our money into I believe that we need to bring it back to Patient Physicians yeah for sure and that's inir primary care we work directly for our patient in the insurance model a doctor doesn't actually work for the patient you work for who pays you right right so in the insurance model I work for Medicare or I work for Blue Cross or Harvard Pilgrim I don't work for the patient my husband recently had an interaction with a doctor in the um er and he said uh just pretend we're not even using our healthc care here what is this going to cost I I can't tell you that you should have seen it it was like he asked for swear words no it's they were appalled to see what what is the actual cost of this right he asked it is it's Healthcare in general healthcare workers we need to understand the costs better if this system is going to survive in this country I named the new practice very Direct Care Veritas is Latin for truth or transparency I think we need to be truthful with our patients about money yeah that you know in the end of the day we we it is a business we get it I mean but that you're doing a job we're we need your services it is an exchange here Mari says $1,000 a month with a huge deductible is what she's paying right now just for her right now my my own family it's I I pay more than that for my health insurance and we have we have a high deductible and um and we are paying just as a society it's my opinion that we're all paying too much for our health insurance for what we actually get in return well that's exactly it it's not that we're not paying enough for for but we're not getting what we're paying for which is aggravating we still need health insurance right you never know the catastrophic things that can happen in life you don't want to bankrupt your family if you end up in the Intensive Care unit uh but what we're all paying I mean I I know multiple um small business owners on the cape that are trying to take care of their employees and paying well over $3,000 a month yeah navigating that system is not for the week I will tell you that right now and so um we are trying to simplify things in our practice by working directly for the patient so indirect Primary Care it's a membership model um basically One Price people can either pay annually or pay monthly but one price you get everything that we do in our practice so talk about the prices because before it's not really a sticker shock I feel like it really is something that is Affordable to everyone so um it's a a monthly rate what as I said the most important thing in Primary Care is the uh relationship and that's really the most important thing and so a patient when they sign up for our practice will pick their own what we call a designated provider their own healthc care provider who's going to be their quarterback uh this person is going to know them inside and out they're going to do the majority of their office visits they're going to be you know hopefully on a firstname basis with their patient yeah and vice versa have more direct contact that than patients are used to and that's the person you you'll pick that quarterback to be your provider you still have access to the whole practice which by the way includes two doctors two nurse practitioners to Pas you've got five RNs and you just added a couple of Specialists or Ortho and also endocrinologist so I mean that's pretty extensive of a network that you've got to just be able to call and be like this is a problem and right now with the lack of Endocrinology on Cape Cod anywhere really if I to refer out to an endocrinologist it's an 11mon weight yeah we just hired Dr Jennifer Patton who's a wonderful endocrinologist she's going to be right down the hallway that's really really nice and so uh and Dr Bill Manning is an orthopedic surgeon wonderful wonderful orthopedic surgeon uh he's not going to be operating but what he's going to do is help evaluate our patients we see a lot of knee pain and hip pain and back pain on Cape Cod we can get an evaluation promptly to help guide how to properly take care of the patient hopefully keep them out of the operating yeah well exactly because if you don't have to cut in there you don't want to and if you can work around it then yeah you want that we also um we also have a a a pharmacist in our practice that's really awesome so we are not a pharmacy but he he is a doctor of Pharmacy his name is Mike and he helps evaluate all of our patients who are on complex medications or uh taking a lot of supplements and helps he's going to be working very closely with Dr Patton to to uh help care for all of our diabetic patients well that's what I was going to say chronic Health Care people this is probably just amazing for and and the other the nice thing about having a practice pharmacist he knows how to save people money yeah the first uh a few dozen people who signed up for the practice a few months ago signed up because Mike was saving them the money that was going to help pay for their yeah I had an instance where my girlfriend Julie had a lapse in coverage and to get her regular prescribed medication it was a mortgage payment that month and she had to make it it's her regular medication so it was ridiculous but I mean she had a laps in coverage and that's what happened she didn't plan for it and I will confess something in all the years of schooling and training I went to they didn't I didn't go to one class about how to help somebody get their medications covered by their insurance program right Mike knows how to do that see that's really nice to have at your fingertips in your disposal too and I just really feel like afford it it affordable I mean for what 1,500 to three grand a year really when you look at it on the larger scale for your monthly fees it's actually less see so our our um memberships start it depends on who you pick to be your quarterback but if you pick one of our wonderful nurse practitioners or PAs it starts at $65 a month the uh we do have an aged hearing system um but we got to talk about that because I K I was at a specific event where I was uh talking to some other of the the older women and boy they got a lot of questions huh they definitely though question know right exactly so what do you say would be the ideal candidate if you're like looking at your your person that would get the most benefit okay so first of all anybody right exactly because I think everybody deserves good primary care I think I'm telling you my husband was like we got to we got to talk about this because I think we need to do this he's also a worry wart uh and a hypochondriac so I don't know if you really want him as your patient sign up today uh we H so really it's any patient yeah and um because everybody deserves good Primary Care everybody deserves to have a relationship with a doctor I realize that there a big question I get all the time is oh I only come in once or twice a year why would I sign up for this talk to us about that person we say that's the goal we only want we want you to be healthy that's where we want to want you to get to that's where we want to keep you um it is up to us to make sure you feel the value of the program what we in addition to building the the strong relationship with a provider uh we have access to our practice where we answer the phone we take our we take a lot Pride we answer the phone we have a wonderful team of people uh so that you can get through to us we pride our our El on creating access so that we can get people in same day next day not it is horribly inappropriate if you've got back pain to wait three weeks to see your primary care doctor which is we've all been there which is why way too many people are getting turfed to urgent care or the emergency room that's when the costs start to Skyrocket um so we pride ourselves on creating access um so that when you can call we want to be there for you we want to take care of you um and so though the healthy person who comes in once or twice a year you never know when things are going to pop up but it's nice to have your Labs it's nice to have your well visits it's nice to have all of that with that primary care doctor that's like it's been a while I haven't seen you for this or hey it's been a while those reminders because when and I tell this people in it in all the stuff that I do well it worked yesterday well it's not working today so we need to fix that and that I'm sure you get that a lot well I've been healthy up until today and now I'm not this is a problem and that's when it's important to have that solid relationship somebody that knows you that you trust yeah so that you can move forward um we uh you mentioned uh having Wellness Labs we do have a lab uh inhouse which is so amazing good morning Dave we're talking to Dr Don uh Dave Dan Arnold from Veritas Direct Care so they've got an in-house lab which is amazing and so we can run 90% of what a patient needs right in house where we can control the costs and their so blood counts metabolic panels kidneys liver electrolytes cholesterol diabetic testing when was the last time you've had any of this done have you been avoiding your Ware I'm not speaking for myself or anything I know I don't even want to know but I need to know so many of the commercial insurance companies we have we've had toughs through the years they don't want to even pay for any routine lab work our lab work is covered by the patient membership so no extra cost there if we do have um more special tests that we need to send out to Quest we have just negotiated wonderfully Low Cash rates for tests that's nice uh with Quest can you work with your insurance for that portion of your care we could still work with people's insurance for everything we don't have to put it on practice yeah we can work with your insurance for everything outside of our practice a big uh big myth that we're not able to we're able to okay um and uh but we can help people save money yeah I mean just yesterday on the news uh it was announced that uh Shields is no longer going to be accepting Blue Cross Blue Shield for this is scary because how much money do you spend for that premium po the with Blue Cross and Blue Shield I know that we paid for it for 20 years and we felt like we had to go to those special doctors all of the time but we never developed a relationship or a followback with those doctors so we had you know the the problem was fixed but there were like questions that happen afterwards and we never really had anybody to follow back with right you need a strong relationship with somebody that you know and trust and that's what we want to develop with all the extra Services you know it's how wonderful it is that we have an orthopedic surgeon sitting down the hallway we have an endocrinologist sitting down the hallway uh we need to get a blood test we can get it right down the hallway um it's it's I think it's going to be Wonder ful for patients you know who it's also wonderful it's going to be wonderful for us yeah healthcare workers do not want to go as fast as they're going right when you add up you can people can look this up online when you add up what Physicians get paid by say Medicare we are supposed to spend in the insurance model supposed to spend less than seven minutes per office visit is not appropriate you can't take care of a human being in seven minutes um that Physicians nurse practitioners Pas don't want that we want to slow down we want to do a better job taking care of our patients I want to do a better job taking care of my patients so this model this model is is is the way it's it's helping us to get in that direction of just doing a better job it's what we want so it's better for us it's going to be better for patients yeah for sure and then better for the community if we can get people get people healthy what are the common questions that people ask you I know I was looking through through your FAQs but what's the difference between Direct Care and concierge medicine so in direct primary care we work directly for the patient it's a membership model one flat fee for per month um in concierge practices and I have nothing against concierge practices whatever works for it's a service right if it works for the patient if it works for the the health care provider I'm all for it Lord knows we need more primary care but in conier models uh uh there's a membership and the U Practice still bills your medical insurance okay so you still have to have a co-pay and deductibles and co- Insurance all of that still applies in addition to the membership I fundamentally just disagree with that and I think that we need to to yeah because it's double dipping it's like again whatever works for them everyone's gonna pay for what they want to have so I in in our model we work directly for the patient yeah I think that's the most important thing um so what happens we go to you you do our Labs we're going to need more Labs we're going to need more this how do you stay in touch with the patient as maybe this care is beyond something you can do sure so uh we'll have the access it's my job running the practice to make sure that everybody's uh panels of patients are at an adequate size that they will still have access and the ability to take care of their patient good so that's my job to be managing all of that um I I don't want to get to a point where people are too busy to see their patient so that's something I'm working on every single day um but we since we'll have the access we can arrange follow-ups and we work for the patients so whatever follow-up plan is right for the patient we will do and if you're out there and you've got a question for Dr Dan about uh the direct care please drop it in the chat and we'll answer it too so it's your job you're managing these doctors making sure that no one's got too much on their plate right in the in in the Insurance in the insurance world uh if you have a test done you want to get go over your result generally you have to have an office visit booked to go I can't stand that so I feel like it's always double dipping on the co well you gotta come in you gotta come in well in the insurance world the visit is the only way anybody gets paid okay in this model it's a flat membership doesn't matter if I see you one time a year or 20 times a year it's the same membership we're going to do whatever is right for you so that cut on nonsense visits which saves your time and your time and if you you have a you want to go over your ultrasound results of just going over to on the phone really quickly is the right thing to do then that's what we'll do yeah if bringing you into the office to pull it up on the big screen and look at the pictures together is the right thing to do then that's what we'll do okay we're also setting up um better more direct communication models so not only do we answer the phone but uh you'll be able to get through by text to your provider email to your provider uh um I have a a wonderful nurse Bethany EML who is uh she's also training to be a nurse coach help people with lifestyle management and she is routinely texting my patients I love that though I mean that's really what it is and sometimes in a lifestyle management situation you need that champion you need that person that checkin that accountability that's really nice to have she's really doing her best to try to coach me uh hey we're all in and pursuit to be better Dr Dan The Journey right exactly trust the process I feel like I'm about the bachelor right now so uh you know we're just we're trying to do a better job we wanna we want to just do a better job for our patients slow down spend more time have access and offer great Services what about um I'm trying to think of other things that I've got questions I know I had a million in five of them for Dr Dan and now I'm like I will tell you a question I hear of a lot is this is money I can't afford it what what can I do so uh we have just uh announced and we're going to be formally rolling out a referral Rewards program so exciting we know that our current patients are a tremendous source of referrals for us yeah we want to reward them so uh in our referral Rewards program if Stephanie if you were a patient of ours and you referred patient and they mention you I Stephanie in the morning sent me we're going to give you a $50 credit right back to your account so one I'm going to need it because my husband's going to be like can I have a I need I need so if you referred one patient a month that's $600 a year back in your account yeah and it works for everybody it helps our business it helps our practice it's going to help you um we this is hot off the presses I'm really excited about this program because I really think it's going to be impactful for our Community we um I I trained at Charity Hospital in New Orleans and Boston City Hospital Boston Medical Center I know that there are people that truly cannot afford a membership with our practice um I also know that we have extremely generous people on Cape Cod that want to help take care of their neighbors so we have just set up a nonprofit um organization called patient supporting patients uh I haven't haded the time I know before you're like oh we don't need another nonprofit I think this nonprofit is needed because you're helping people directly and we don't have a website yet but it is just going to be a method for people to donate money that is going to be used for the primary care expenses for their neighbors and such on Cape Cod maybe not even not even for our practice but for any primary care expenses going to set up a nice easy way for people to apply who truly need financial support important because if you can catch something early then you can stop things from getting way worse and being more costly I mean honestly that's what primary care is is supposed to be doing right right right right is to stop you from having all of those big giant illnesses before you get to where you need all of these crazy Specialists right so so stay tuned for more information about uh patient supporting patients and what if you guys just get too like too busy or you need a vacation how do you deal with those situations so we occasionally I need a vacation we all need our space too very healthy healthcare workers have to take care of themselves um we have to focus on our own health we do uh but we have a big enough practice that we can cover ourselves so yes you'll have a quarterback and I said your quarterback is going to do the majority of your visits and get to know you but if uh Juliana our wonderful PA was your quarterback in the office and she was on vacation you call the office we you still have access to everybody okay so you could still get in to see me so if you had back pain we don't want you waiting around right we'll get you in get your triage get you taken care of but all your information is still there so you're gonna have all of that and we communicate with Juliana to tell her exactly what's going on so she can follow up on the issue when she's back okay um and uh it just it just works I we've can we've been covering ourselves like that forever and uh but again we could just do a better job also growing your practice is not necessarily the worst thing on earth if you got to bring on another doctor you're doing something right right and in this model you know I told you I'm going to be managing panel sizes when the panel sizes get to a certain point it's going to be time to hire another doctor and I will tell you that there are a lot of people a lot of Physicians interested in what we're doing for sure they're like you do it first well watch I'm kidding I'm kidding no I so direct primary care has been around for 25 years in this country it is much more popular out in the west the South is really popular in Maine okay um but you know this being Cape Cod we're very set in our ways it's so true we're like 20 years it behind everybody because we're like nope we're going to do it this way we've always done it this way scking with this it was not it didn't exist I don't know why we're this way but we definitely are so good morning Diane to thanks for joining us chenine Hi how are you Hi Shenene uh and so uh shenin is our one fuler so everybody who joins our practice will get to know Shen's voice I really like that you're knowing everyone I feel like it it builds a comfort factor where you're okay with calling because right now I just feel annoying when I call and I'm like hi it's me again because we've got a large family so it's not always about like Who you calling for which one this time I feel annoying no no no it's not we we it we like taking care of people and uh and so yeah and I want people to be on a first name basis with the people answering the phone and you're not there yet with the under 18 but you're not yet yeah not yet that's exciting news though I know right stay tuned what about um mental health I know it's a huge problem that is it mental health is a huge problem obviously there's we're basically in a crisis with this country um I do believe that more funds need to be directed in in towards mental health care it's you and we're going over the dollars and cents of all of the the medical and and the business portion of being a doctor and how little you guys actually get at the end of the day so for in right for instance in for the average patient in America it is estimated that 80 to 85% of their health care should in an ideal world happen in their Primary Care doctor's office that makes sense right 80 to 85% some people more some people less uh across all payer for all payers across this in the United States in the insurance model Primary Care is only earning 5.6 cents on the dollar um so 5.6% to take care of 80% of the work is just not it's just not appropriate believe me I tried to move that needle over the last few years in Behavioral Health it's only 1.4 cents on the dollar of Health Care expense is actually sent towards Behavioral Health that is why we can't find psychiatrists we can't find good therapists colist we need more of it believe me I would love to bring in do you have a good Referral List is there anything we have some providers we work very closely with um because even sometimes just getting a referral or finding someone that will take your phone call is also difficult but also Primary Care practices 80 to 90% of of Psychiatry right is done in the primary you actually right and if we can slow down and spend more time work with our practice pharmacist who understands the pharmacokinetics of all the different medications we can spend time and and and actually sometimes that's all people really need is to just kind of sit with it and realize bit time yeah yeah yeah but uh would I love to uh would I love to bring a Psy slam that desk slam it I passionate I love it though what I love to bring a psychiatrist psychologist into our model absolutely yeah for sure because it's just necessary but I do love the fact that you've got the time to spend with people people and maybe that's just it they need someone to listen to them well we've got the doctors for you what about people with like previous conditions or prior health issues that are maybe they've got it settled you that's basically 100% of people so you know everybody's got something and uh no I was fired from my my primary care physician which is why I don't currently have one right now because I had a pre-existing condition do you need a business card hello because my insurance stopped taking it and they basically fired me because I had a pre-existing condition so I'm and and I know I'm not the only person out there that is kind of floating with what what what am I supposed to do so it's like people going to just like the small urgy cares for their regular things not appropriate and that's not appropriate for health care it's not it's just kind of where we've got shuffled oursel into at this point in time I think you could ask any of our wonderful nurses any of our team we specialize in people with complex medical issues well so talk to me about that what what is some of the things that you work with I know you said diabetes I saw COPD somewhere I saw a couple of other just chronic conditions right we have a we have a system um set up for people with chronic conditions that's covered by their membership it's called remote patient monitoring RPM uh we there's we take care of a lot of people with chronic congestive heart failure for instance where it's important to keep a close eye on their Vital Signs so with this system the patient goes home with um a a tablet and that only does this program it's and a wireless blood pressure cuff a pulse oximeter and a scale they wake up in the morning go to the bathroom eat breakfast sit down take their Vital Signs and they get beamed right to our office nice we have a team that uh sits there watches these vital signs all day we get alerted if there's changes so if somebody with congestive heart failure starts to retain fluid weight goes up oxygen goes down we can identify that before the symptoms develop put them in the emergency room you can't get that level of care anywhere else you just really you just can't and you know if if you came in to see us we change a blood pressure medication you know why would I only want and see you back in three four weeks only to get that one follow-up blood pressure check no we could send you home with this remote patient monitoring kit where you can check your blood pressure daily for us just for two weeks three weeks we can see if the medication we changed is actually working that's all covered by your membership the phone calls you must get though must be ridiculous like I have this mole will you take a look at it I have this this we take a look at it that's what we do that's what we do and you know and we can we can triage issues we're still we utilize tella Health we utilize it appropriately all the time people take pictures of a mole and text it to us so we can just take a look and say so funny you know oh yeah okay we'll look at that next time you're in the office next month or you better get in here today to look at so you know that's really nice to have as annoying as it is I'm sure I'm sure initially it's like crazy and then it kind of T tapers off it must well I will say once as you're addressing the issues and and what is that why does it get easier over time when you know your patient it actually gets easier yeah it does it gets easier iier to take care of somebody when you know them uh because you know the shape of the mole on their back you know that their dog died last year and they're still having trouble with it yeah you know that uh you know their mother had ovarian cancer and that's really keeping them up at night because they're approaching the age that yeah we when you know somebody you know that stuff and yes but it takes a while to get to know somebody yeah when our new patients come in I tell them it's going to take you know you're it's a process if you're 68 years old I've got 68 years of stuff to catch up on it's going to take us more than one visit and that's okay yeah um so yeah it's uh it we but it's all about the relationship getting to know our patients you are going to be you're out there in the community you're talking to people you're really just making a push for this right now um now before the end of the year if people swap over I do suggest that you go and talk to Dr Dan if you've got questions if you're like what about this what about this what about this you are a very patient individual father of six to be able to handle all these conversations and questions what's the best way for people to Wade their foot into this maybe it's not for them they don't know yet they're feeling anxious they can call the office talk to any of our staff I am very willing to talk to people um we we're willing to give people tours of the office and they can uh you know talk to me for as long as they they want what about home visits so uh home visits uh we uh we do them when it's appropriate okay yeah and uh generally historically if I'm doing a house call that's not usually a good thing for you but we we do whatever is right for the patient yeah I love it um Lynn says primary cares aren't taking new patients due to coid mine quite uh quit the system and I can't find a new one urgent care for me I've been in the same space which is scary given you know previous conditions so uh and it hasn't been a long time I'm okay everyone I'm doing okay just had my checkup um however that's just a scary space for people not to be able to be having a doctor that they can call on for silly things and you're going to urgent care not good no it's not good same day access Shelly says same day yes yeah no we we uh we pride ourselves on that and um I just I I'm looking over your shoulder chenine comments that she loves it when people say it's me again she knows them she's like hey girl hey did you take your medication I love it good morning Dominique thanks for joining us we're business owner that health insurance costs are crippling small businesses in this country and I I would love to start the process of talking to small businesses about how they can potentially look at less expensive Insurance options yeah but still take care of their employees you know business owners that we we want to take care of our employees I really do I swear guys I really do wouldn't it be really nice it's like doctor day once a month doctor's here what's the problem we want our employees to be happy but we can direct primary care practice can work very closely with small businesses to
